Project 16:49 is seeking to fill a Part Time Case Manager position. The Case Manager serves as the primary point of contact for program participants, and is responsible for supporting participants in making specific positive changes according to their individual goals and action plan. The Case Manager ensures participants have access to the services and supports they need to remain in school and graduate, and acquire the independent living skills needed to become self-sufficient adults.
Essential functions of the position are client intake and assessment; resource referral; individual goal planning and progress review; coaching and evaluation; and accurate documentation. The position also is responsible for providing these services within the framework of Project 16:49’s Mission Statement and Policies & Procedures. Frequent travel within Rock County is required.
Position requirements include a Bachelors degree (Masters preferred) in psychology, social work, sociology, child development or related field from an accredited institution, and 2+ years related work experience. Case management experience and/or experience working with at-risk youth, preferably runaway and homeless youth, is a plus. Valid driver’s license, proof of insurance, good driving history, and reliable transportation are required.

Position Description CASE MANAGER
Project 16:49 is a local non-profit organization established to provide Rock County’s unaccompanied homeless youth with safe, stable housing options and case management support focusing on: education, employment, physical and mental health, independent living skills, and community connections. Project 16:49 employs evidence-based assessments and a trauma-informed approach to assist youth with the development of individual goals and action plans.
Within Project 16:49’s positive youth development framework, the Case Manager supports clients in making specific positive changes according to their individual goals and action plan. The Case Manager serves as the primary point of contact for clients, and is responsible for providing, or ensuring the provision of, the services and supports clients need to remain in school, graduate, and become self-sufficient adults and contributing members of the community.
Duties & Responsibilities
- Conduct intake interviews; determine program eligibility; recommend clients for program admission
- Identify and provide appropriate referrals to other community support services to ensure clients’ basic needs are met
- Provide client orientation to the program; explain program rules, expectations and clients’ rights
- Assist clients with the development of individual goals and action plans specific to their needs; conduct strengths and needs assessments
- Support clients’ ability to address barriers and achieve established goals; refer clients to outside service providers, and coordinate services as necessary
- Assist clients in completing high school diploma or GED, applying/registering for further education, obtaining employment, securing permanent housing, learning independent living skills, and developing other community connections
- Monitor progress through regular client contact and meetings; maintain client files and supportive documentation chronicling progress
- Provide aftercare follow-up contact and services
- Ensure accurate tracking and documentation of all referrals and services provided
- Ensure case records, files and data entry programs are maintained in a proper manner and in accordance with the confidentiality policy, and that all statistical information and data required by any government agency or funding source is completed and submitted on time
- Act as an advocate for clients’ best interests
- Establish and maintain strong working relationships with local service providers
- Supervise interns and volunteers involved with program service delivery
- Collaborate in a team approach to develop, improve and/or expand programs and services
- Provide program reports for executive director, review team and board
- Attend meetings and trainings, as required
- Participate in outreach, community education activities and key organization events
- Perform other duties as needed
