Services & support for youth experiencing homelessness
Project 16:49’s community care and outreach services provide basic needs assistance, referrals and optional case management for unaccompanied homeless youth ages 14-24, primarily high school students.
Assistance includes, but is not limited to, any or all of the following:
- Emergency food and Foodshare enrollment
- Access to physical and/or mental health care and health insurance
- Identification of safe housing options
- Obtaining personal identification documents (birth certificate, social security card, state ID/drivers license) so youth can access benefits and employment
- Hygiene supplies, school supplies, clothing, winter gear
- Referrals to other appropriate programs, based on individual situations and needs
- On-going regular contact, guidance and support
- Assistance with goal setting, life skills, education and employment
