Community Care & Outreach

Services & support for youth experiencing homelessness

Project 16:49’s community care and outreach services provide basic needs assistance, referrals and optional case management for unaccompanied homeless youth ages 14-24, primarily high school students.

Assistance includes, but is not limited to, any or all of the following:

  • Emergency food and Foodshare enrollment
  • Access to physical and/or mental health care and health insurance
  • Identification of safe housing options
  • Obtaining personal identification documents (birth certificate, social security card, state ID/drivers license) so youth can access benefits and employment
  • Hygiene supplies, school supplies, clothing, winter gear
  • Referrals to other appropriate programs, based on individual situations and needs
  • On-going regular contact, guidance and support
  • Assistance with goal setting, life skills, education and employment
